    Please     include the following information in your message:
    -What your treatment needs are (i.e., anxiety, trauma history, relationship struggles, grief)
    -When your available times are to meet between Mon-Thurs (i.e., available before 3pm except on Friday)
    -The name of your insurance provider or if you are interested in self-pay (i.e., Anthem, Aetna, UBH)
